What happens during an assessment?
During the assessment, we'll talk with the young person and their parent/carer to understand their experiences, strengths, and challenges. We use evidence-based tools and take a collaborative approach.
Following the assessment, we'll work together to develop a formulation - a shared understanding of what's happening and why. This helps us create a personalised care plan.
